HIPAA Privacy Notice – Full Policy

We respect your privacy and protect your personal health information (PHI). Your medical records will only be shared with those directly involved in your care, or as required by law.

    • You may request a copy of your medical record.
    • You may ask us to correct information you think is wrong.
    • You may ask us to limit who sees your information.
    • You may request we contact you in a specific way.
    • You may request a list of who we have shared you PHI with.
    • We will notify you if your information is ever compromised.
    • We will never sell or share your PHI for marketing purposes.
    • You may file a complaint if you think your privacy rights were violated.

 

Insurance Consent – Full Policy

We work directly with your insurance company to process claims and receive payment for services provided. Certain information may be shared with your insurer as needed for billing and verification.

Your Responsibilities: 

  • You authorize us to release necessary medical information to your insurance company for claim processing.
  • You are responsible for any copayments, deductibles, or non-covered services.
  • You agree to notify us of any changes to your insurance coverage.


 

Financial Policy Full Policy

We are committed to transparent, fair billing practices.

  • Insurance: If we are in network, we will bill your insurance directly, if not, you may request a superbill to submit to your insurance provider.

  • Self-Pay: Services are paid directly by patients/families at the time of care at the time of service or through a paid subscription.
  • Payment methods: Cash, credit/debit cards, and HSA/FSA cards are accepted.

  • Refunds: If services are prepaid but not rendered, refunds will be processed promptly.


 

Patient Rights & Responsibilities – Full Policy

You have the right to:

  • Respectful, safe, quality, compassionate care.

  • Understand your care plan in plain language.

  • Participate in your care.
  • Ask questions, request a second opinion, and raise concerns without fear.

  • Financial transparency.
  • Confidentiality of your medical information.

You are responsible for:

  • Providing accurate health information.
  • Following care plans as agreed.
  • Ask questions when you don’t understand.
  • Treating staff with respect.
  • Meet financial obligations for your care.

Consent to Wound Care Treatment – Full Policy

Your provider will explain your treatment options, including procedures such as debridement (removal of unhealthy tissue), dressing changes, diagnostic tests, medications, and other wound care treatments.

  • Benefits: Treatment can help promote healing and lower the risk of infection and amputation.

  • Risks: All treatments carry risks, such as infection, bleeding, pain, scarring, or slow healing. No outcome can be guaranteed.

  • Alternatives: You may refuse treatment, continue care with another provider, or choose no treatment at all. Each choice carries its own risks and benefits.

  • Wound Debridement: This procedure removes dead tissue to improve healing but may temporarily make the wound larger and carries risks such as bleeding, infection, or pain.

  • Photography: We may take photos of your wound(s) to track healing, improve care, and support education. These images are protected health information and handled securely under HIPAA.

By continuing with treatment, you acknowledge that your provider has answered your questions and that you understand the risks, benefits, and alternatives.


Telehealth & Communication Policy  – Full Policy

Advanced Wound Care Services ensures secure, HIPAA-compliant communication with patients. Key points include:

  • How we communicate: phone, SMS/text messaging, email, and secure patient portal

  • Types of messages: appointment reminders, follow-ups, order alerts, and educational updates

  • Consent requirements for SMS/text messaging

  • Patient rights regarding electronic communications and privacy protections
