Healthcare Professionals Submitting a Referral

ReReferring a patient to Advanced Wound Care Services is simple and secure.

Step 1: Complete the Referral Form

Submit the online referral form below. Please be sure to include:

  • Patient demographics and contact information

  • Relevant wound history and diagnosis

  • Recent progress notes (if available)

  • Insurance information (if known)

Step 2: Submit Supporting Documentation

You may send clinical notes, imaging reports, and recent wound measurements directly with the referral form. Documents can be securely faxed, emailed or called in to our office.

Step 3: Clinical Review & Triage

Once a referral is submitted, our team will take the following steps:

  • Referral Review: All referrals will be reviewed promptly.
  • Patient Outreach: Our clinical team will contact the patient/caregiver directly to gather additional information, answer questions, and schedule an initial evaluation.
  • Care Coordination: Our provider will conduct an initial assessment, create an individualized wound care plan and provide education and resources. We will share this evaluation and plan with the referring provider and other specialists involved in the patient’s care.
  • Ongoing Communication: Referring providers and facilities will receive updates, including significant changes in status, recommendations for follow-up, and additional care coordination when appropriate.
  • If Additional Information Is Needed: Our team will reach out for clarification, records, or supporting documentation.
