This new patient information page will guide you through what to expect and how to get started, whether you’ve been referred by a healthcare provider or are seeking care on your own.

New Patient History

Before scheduling your first visit, we’ll need to gather some basic information to help us understand your wound care needs. If your provider has already submitted a referral, we’ll review the information they shared and contact you if anything else is required.

We require the following:

  • Your insurance card and photo ID
  • Referring provider/organization contact information
  • Contact and demographic information
  • Medical history and allergies
  • Recent hospitalizations or procedures
  • Current medication list
  • Recent wound care records and treatment plan (if applicable) 

Insurance & Billing

We accept most major insurance plans and will provide clear information about coverage and any out-of-pocket costs prior to your first visit. Concierge and Direct-Pay options are also available. Contact us for pricing and availability.

If you’re a provider or healthcare facility, please visit our Referrals Page to begin the referral process or join our Referral Network.

Preparing for Your First Visit

  • Prepare a clean space for treatment and ensure good lighting.

  • A caregiver or family member is welcome to be present.

  • We bring all necessary supplies and equipment to your home.

  • If you experience pain during wound care, take your prescribed pain medication approximately 30–60 minutes before your scheduled appointment, unless otherwise directed by your healthcare provider.

What to Exect

From your first visit through ongoing care, Advanced Wound Care Services supports you with personalized treatment, clear guidance, and home-based services designed for comfort and convenience.

Referral or Intake and Initial Evaluation

We receive your referral or contact form. Our team reviews your case, ensures we have all the information needed, and calls you to schedule a home visit. You will also need to review and sign our Patient Policies.

First Visit

Our licensed specialist will arrive at your home and perform a full assessment, evaluate and treat your wound(s) and create a care plan that best fits your needs. You will receive instructions, education and basic supplies to get you started.

Ongoing Care

After your first visit is complete, an order will be placed for supplies and any additional recommended treatment, testing and referrals. Follow-up visits will be scheduled as needed to support healing and monitor your progress.

Communication

Our provider/wound care team will stay in touch with you throughout treatment. We will also communicate with your other providers ensuring everyone stays up-to-date with your progress and care plan.
